Gallbladder cancer is rare in most of the world, but the Gangetic belt of India — Uttar Pradesh, Bihar, West Bengal and Assam — has some of the highest rates anywhere. Women are affected more than men. Because early gallbladder cancer usually causes no symptoms, many people are diagnosed late. Knowing the signs and asking the right questions about gallstones can make a real difference.

What is the gallbladder?

The gallbladder (pitt ki thaili) is a small pouch under the liver that stores bile, a fluid that helps digest fat. Bile flows through tubes called bile ducts into the intestine. A tumour here can block these ducts and cause jaundice.

Who is at higher risk?

  • Long-standing gallstones, especially large stones present for many years.
  • Women above 40.
  • Living in the Ganga basin; researchers are studying water, soil and diet factors.
  • Gallbladder polyps larger than about 1 cm.
  • Chronic infection such as typhoid carrier state, obesity, and family history.

Most people with gallstones never get cancer. But if you have stones and symptoms, talk to a surgeon about whether removing the gallbladder (cholecystectomy) is advised.

Symptoms

  • Pain in the upper right side of the tummy that keeps coming back.
  • Yellow eyes or skin (jaundice), dark urine, pale stools, itching.
  • Loss of appetite, nausea, and weight loss.
  • A lump or swelling in the upper abdomen.
  • Unexplained fever.

These can also be caused by stones or infection. If jaundice appears, see a doctor the same week. See also: unexplained weight loss.

Important: ask for the removed gallbladder to be tested

Some gallbladder cancers are found by chance after a routine stone operation. Always ask that the removed gallbladder is sent for histopathology (biopsy) and collect the report. If cancer is found, a second, bigger operation may be needed soon.

Tests used

  • Ultrasound of the abdomen — first test; shows stones, thickening or a mass.
  • Contrast CT or MRI/MRCP — shows spread to liver, bile ducts and lymph nodes.
  • Blood tests — liver function tests, and tumour markers like CA 19-9 and CEA. Markers alone cannot diagnose cancer. See tumour marker tests.
  • Biopsy — usually an image-guided needle biopsy, needed before chemotherapy.
  • PET-CT — sometimes used to check for distant spread. See PET-CT guide.

Treatment options

  • Surgery — the main treatment for early disease: removal of the gallbladder with part of the liver and nearby lymph nodes (radical or extended cholecystectomy). It should be done by an experienced GI or HPB onco-surgeon.
  • Chemotherapy — after surgery to lower the chance of return, or as the main treatment when surgery is not possible.
  • Radiation therapy (sikai) — in selected cases, sometimes with chemotherapy.
  • Stenting for jaundice — a small tube placed in the bile duct by ERCP or PTBD to relieve blockage, itching and infection.
  • Targeted therapy and immunotherapy — for some advanced cancers, based on biomarker tests such as HER2 or MSI.
  • Palliative care — to control pain, itching and nausea. See palliative care.

Questions to ask

  • Is the cancer operable? Has an HPB onco-surgeon reviewed the scans?
  • Do I need a stent before chemotherapy?
  • What is the written estimate, and is it covered under Ayushman card?
  • What follow-up tests will I need, and how often?

Cost and support

Surgery and chemotherapy for gallbladder cancer are included in Ayushman Bharat PM-JAY packages at empanelled hospitals. Ask for a written estimate first. See cost and written estimate and financial help in Uttar Pradesh.

Frequently asked questions

Pitt ki pathri se cancer hota hai kya?

Most people with gallstones never get cancer, but long-standing stones raise the risk. Discuss with a surgeon whether removing the gallbladder is advised.

Is gallbladder cancer curable?

When found early and fully removed by surgery, some patients are cured. Many are diagnosed late, when treatment aims to control it and relieve symptoms.

Why is gallbladder cancer common in UP and Bihar?

The exact reason is not proven. Gallstones, water and soil factors, infections and diet are being studied.

Is CA 19-9 enough to diagnose it?

No. It can rise in jaundice or infection too. Scans and a biopsy are needed.

What should I do after a gallstone operation?

Collect the histopathology report of the removed gallbladder. If it shows cancer, see an onco-surgeon quickly.

You don’t have to face this alone. Ask Disha (chat on this page), call +91 99307 09000, or WhatsApp us. For hospital care in Uttar Pradesh, see ZenHeal Varanasi and ZenHeal Prayagraj.
