Cancer Biomarker Testing in India: Questions to Ask Before Paying

A recommendation for a cancer biomarker test can arrive with unfamiliar names, several package options, and a substantial bill. Before paying, ask a practical question: what treatment decision could this test change for me now? The answer should come from your oncologist, with input from the pathology team when needed. This guide helps patients and caregivers in India discuss usefulness, sample requirements, costs, and results without choosing treatment from a laboratory report alone.

Know what the test is looking for

Biomarkers are features such as genes or proteins that provide information about a cancer. Certain tests help clinicians choose targeted medicines or immunotherapy. Tumour profiling is not the same as testing for inherited cancer risk, although a tumour result can sometimes prompt separate inherited-risk assessment. A matched treatment is a possibility, not a promise that it will work. National Cancer Institute explanation of biomarker testing.

Testing is routine in some situations and less useful in others. Examples include hormone receptors and HER2 in breast cancer, or certain gene changes in non-small cell lung cancer. These are examples, not a shopping list for every patient. The right tests depend on the cancer and the decision being made. American Cancer Society overview of biomarker tests.

Clarify the sample and the limits

Ask whether stored tumour tissue is suitable, whether enough remains, and who will arrange its transfer. Depending on the cancer and test, testing may use tumour tissue, bone marrow, blood, or another sample. Samples may need to go to an outside laboratory, which can affect the timeline. Ask for the expected report date rather than assuming all tests return equally quickly. How biomarker samples are tested.

A blood-based liquid biopsy does not automatically replace tissue testing. A negative result may not exclude a relevant tumour change. For example, the FDA’s information for one approved liquid-biopsy test specifically describes this limitation and the possible need for tissue confirmation. Ask your oncologist how a negative or inconclusive result from the proposed test would be handled. FDA information on liquid-biopsy limitations.

Ask these questions before agreeing to payment

Will the result change a decision?

  • What is the exact test name, and which clinical question should it answer?
  • Is it needed before the next treatment decision, or could it be considered later?
  • Why is this single-marker test or larger panel appropriate for my situation?
  • Have any of these tests already been completed? Can existing results avoid duplication?
  • What happens if no useful finding is identified?

Who will perform and interpret it?

Request the testing laboratory’s name, not only the sample-collection centre’s name. In India, you can check a laboratory’s current NABL accreditation status and published scope using the official NABL laboratory directory. Ask whether the relevant test is covered. Accreditation is a quality consideration, not proof that a particular test will benefit you. Confirm who will explain the report and integrate it with your other records.

What is the complete financial commitment?

  • Request an itemised written quote in rupees, including sample collection, tissue preparation, transport, testing, and interpretation where charged.
  • Ask what happens financially if the sample is insufficient or the test fails: is repeat testing included, and what is refundable?
  • Check the exact test with your insurer or applicable scheme before payment. Ask the hospital to clarify required approval and documents.
  • If a relevant finding appears, ask about the medicine’s availability in India, expected treatment and monitoring costs, and practical access.
  • If a trial is discussed, ask the trial team about eligibility, travel, and which costs are covered. Do not assume participation or funding.

Read the result with your treating team

A report may identify a potentially useful treatment match, no currently useful finding, or a variant of uncertain significance. An uncertain variant should not be treated as an established treatment target. Even a relevant finding may not lead to an accessible or effective therapy. Cancer biomarkers can also change over time, so repeat testing may sometimes be considered. NCI guidance on interpreting results and limitations.

Ask the oncologist to distinguish an established option for your cancer from off-label use or a research option. Discuss expected benefit, alternatives, and side effects. Targeted medicines can cause significant adverse effects; a precise-sounding test does not make treatment risk-free. NCI information on targeted therapy.

Keep a short decision record

  • Exact test and laboratory, sample type, and report due date.
  • The treatment decision it is meant to inform.
  • Total quote, coverage confirmation, and failed-sample policy.
  • Date of the results discussion and questions still unanswered.

Agree on care while you wait

Ask the oncology team what care should continue while results are pending and which symptoms require immediate contact. Do not postpone or change recommended treatment yourself while shopping for tests.

Make the next conversation specific

Take the proposed test name and written quotation to your oncologist before committing. This educational guide supports that discussion and cannot determine which test or treatment you need.
